We are seeking a highly skilled Clinical Pharmacist to join our team at Northern Health Authority. As a key member of our pharmacy department, you will be responsible for providing exceptional patient care and contributing to the development of our clinical services.
Key Responsibilities- Assess and recommend individualized treatments for patients in our hospital setting.
- Act as the primary resource for drug information, education, and expert advice to healthcare professionals.
- Monitor drug prescribing, distribution, and therapy to ensure safe, effective, and rational use of medications.
- Maintain accurate records and statistics related to workload and patient care.
- Baccalaureate degree in Pharmacy and current licensure with the College of Pharmacists of British Columbia.
- Completion of a Hospital Pharmacy Residency Program or a combination of at least one year of hospital experience and advanced education courses.
- Active membership in the Canadian Society of Hospital Pharmacists is encouraged.
- Assessment and Treatment: Demonstrated ability to complete initial and ongoing patient assessments through appropriate technical, therapeutic, and safety interventions.
- Teaching: Ability to teach patients and others both one-on-one and in groups.
- Knowledge Integration: Integrates best practice and current research evidence to support professional practice decisions and actions.
- Communication: Demonstrated ability to communicate effectively with patients, families, and healthcare professionals using verbal, written, and computer communication means.
- Critical Thinking: Demonstrated ability to integrate and evaluate pertinent data to problem-solve and make decisions effectively.
- Human Caring and Relationship Centered Practice: Ability to promote patient-centered care that demonstrates care for and with patients and families, sensitivity to diverse cultures and preferences, patient advocacy, and social justice concerns.
- Management: Demonstrated ability to organize work, set objectives, and establish priorities.
- Leadership: Promotes staff morale, engagement, and empowerment.
- Teamwork: Demonstrated ability to foster teamwork and a commitment to excellence in the provision of patient care.
