Pharmacist Clinical Practice Lead

We are seeking a highly motivated and experienced Pharmacist Clinical Practice Lead to join our Pharmacy Services team at Humber River Health. This is an exciting opportunity for a passionate healthcare professional to lead and contribute to the development of clinical pharmacy practice, quality initiatives, and education programs.
Job SummaryThe successful candidate will be responsible for supervising and coordinating clinical activities of pharmacists, providing oversight of clinical pharmacy practice programs, medication use systems, and quality initiatives. They will work closely with the Director, Pharmacy Service, to ensure the provision of high-quality pharmaceutical care and alignment with evidence-based best practices and regulatory standards.
Responsibilities- Develop and implement clinical pharmacy practice standards and procedures
- Coordinate patient care activities and oversee all clinical activities of the department
- Proactively promote cost-effective, high-quality care through the development and implementation of clinical initiatives
- Maintain and evaluate staff and program compliance with professional best practices and standards
- Participate in departmental and hospital programs directed at improving patient care standards
- Entry-level Doctor of Pharmacy degree from a recognized school of Pharmacy or equivalent education
- Minimum 5 years of Pharmacist experience in a hospital setting required
- Experience in a direct patient care environment (Hospital Pharmacy) within the previous 5 years
- Registration in good standing with the Ontario College of Pharmacists – Part A (Pharmacists providing direct patient care)
Humber River Health offers a competitive salary range of $120,000 - $180,000 per annum, commensurate with experience, as well as a comprehensive benefits package and opportunities for professional growth and development.
