Job Summary

Community Therapists is seeking an Occupational Therapist to join our team in a community-based practice. This role will appeal to OTs seeking a diverse and challenging practice serving clients across the rehabilitation continuum.

Key Responsibilities

  • Conduct hospital discharge planning and assessments to ensure seamless transitions for clients.
  • Develop and implement cognitive assessments and rehabilitation plans to increase client independence.
  • Design and implement graduated activity plans to enhance home and community functioning.
  • Develop and supervise community integration plans to promote client participation and engagement.
  • Provide case management services to facilitate client access to other health services.
  • Liaise with funders, family, and healthcare teams to ensure effective communication and coordination.

Requirements

  • Member of COTBC or eligible for registration.
  • Driver's license and access to a reliable vehicle.
  • Excellent verbal and written communication skills.
  • Aptitude for negotiation and consensus building.
  • Experience working with insurance fee payers is an asset.
  • Experience working with clients with TBI is an asset.
