Job Summary
We are seeking a skilled Community Liaison Worker to join our team in Richmond, BC. As a Community Liaison Worker, you will provide support services to clients and their families in community health, working across multiple sites, clinics, and services. You will liaise and coordinate services with medical professionals within an integrated health service delivery model and a range of programs.
Key Responsibilities
- Provide support services to clients and their families in community health
- Work across multiple sites, clinics, and services, liaising and coordinating services with medical professionals
- Support clinical staff as a member of healthcare teams by providing appropriate information and support related to clients with complex healthcare needs
- Monitor clients, document and provide verbal reports on observations to clinicians in accordance with standard procedures or specific direction
- Act as an advocate and/or a liaison for clients, facilitating access to community services and resources
Required Skills and Qualifications
- Social Services Certificate or Diploma from a recognized College, plus (1) year recent experience in a community setting supporting clients within the relevant client population, or an equivalent combination of training and experience
- Valid BC Driver's License and access to personal vehicle
Salary and Benefits
The estimated salary for this position is $31.56-$33.28 per hour, based on the job requirements and location. We offer a comprehensive health benefits package, including MSP, extended health and dental, and a municipal pension plan. About the Role
This is a casual position, with variable shift times and days off. The successful applicant will be required to work independently and in cooperation with others, communicate effectively, and establish/maintain satisfactory work relationships with the public and other employees.
