As an Occupational Therapist with CBI Home Health, your responsibilities will include providing OT interventions, including ADL/IADL strategies, equipment recommendations, and home modifications. You will also collaborate closely with an interdisciplinary team to develop tailored care plans and interventions that empower patients to reclaim their independence and thrive in their daily lives.
Key Responsibilities:- Assessment & Implementation: Provide OT interventions, including ADL/IADL strategies, equipment recommendations, and home modifications.
- Team Collaboration: Use a collaborative, interdisciplinary approach to care and services.
- Holistic Care: Support a restorative approach to health, offering interventions, health coaching, and self-management support.
- Digital Health: Utilize our digital platforms for care delivery, collaboration, documentation, and intervention.
To be successful in this role, you must be a registered member in good standing with the College of Occupational Therapists of Ontario. You should also possess excellent communication skills and be known for your ability to exercise good judgment and critical thinking in client care decisions. A valid driver's license, vehicle, and car insurance are also required for travel between client locations.
