**Job Summary**
We are seeking an experienced Occupational Therapist Assistant and Physiotherapist Assistant (OTA/PTA) to join our dynamic and self-directed care team at CBI Home Health. As an OTA/PTA, you will play a vital role in providing care to patients in their own homes, using your skills and expertise to help them achieve their goals and live independently.

**Key Responsibilities**
- Provide occupational therapy and physiotherapy services to patients in their homes
- Work collaboratively with an interdisciplinary team to develop and implement individualized care plans
- Use assistive equipment and mobility aids to support patients in achieving their goals
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination
- Participate in ongoing learning and professional development to enhance skills and knowledge

**Requirements**
- Post-secondary diploma as an Occupational Therapist Assistant/Physiotherapy Assistant (OTA/PTA)
- Valid driver's license, vehicle insurance, and reliable vehicle required
- General knowledge of mobility aids and assistive equipment
