Key Responsibilities:

- Provide comprehensive nursing assessments and develop individualized care plans for patients

- Conduct home visits to assess patients' needs and provide education on self-care and management of chronic conditions

- Collaborate with healthcare professionals to coordinate care and ensure seamless transitions between care settings

- Develop and maintain relationships with patients and their families to provide emotional support and guidance

Requirements:

- Master's degree in Nursing or related field

- Current registration with the College of Registered Nurses of Manitoba

- Experience working in home care or similar setting

- Strong communication and interpersonal skills
