Job Title: Pediatric Psychiatric Liaison Nurse

Job Summary:

We are seeking a highly skilled and compassionate Pediatric Psychiatric Liaison Nurse to join our team at Fraser Health Authority. As a key member of our interprofessional care team, you will play a vital role in providing person-centered and recovery-oriented care to pediatric patients with psychiatric and concurrent disorders.

Key Responsibilities:

  • Perform psychosocial assessments and collect collateral information to determine service priorities and develop care plans.
  • Provide crisis intervention and counseling services to patients and their families, including short-term follow-up treatment and case management.
  • Collaborate with the interprofessional care team to ensure effective patient flow and discharge planning.
  • Communicate and consult with Coordinators and other members of the interprofessional mental health and substance use team on patient care issues, resources, and discharge planning.
  • Document assessments, care plans, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.

Requirements:

  • Graduation from an approved school of Nursing or Psychiatric Nursing.
  • Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • One (1) year's recent, related experience in an acute psychiatric setting, including experience working with an adolescent population, specialty training in adolescent psychiatry, or an equivalent combination of education, training, and experience.
  • Valid BC Driver's license and access to a personal vehicle with appropriate business insurance.
