About the Role

The Nova Scotia Health Authority is seeking a skilled Health Care Social Worker to join their team. As a member of the interdisciplinary healthcare team, the successful candidate will provide collaborative, holistic care on the primary to quaternary care continuum.

The Health Care Social Worker will be primarily concerned with the relationships between people and their environments, which are integral to psychosocial, emotional and spiritual wellbeing. The social determinants of health are embedded in social work theory and practice and are the focus for the dynamic and interactive process of engagement, assessment, planning, intervention and evaluation.

The ideal candidate will have a Bachelor's degree in Social Work from a recognized university, current registration with the Nova Scotia College of Social Workers, and 2 years' post BSW related clinical experience. Previous experience working in community-based settings is preferred.

Key Responsibilities

  • Build upon the strengths of patients/clients by focusing on existing coping strategies and problem-solving skills, past successes, and personal resources.
  • Use a client-/family-centered approach and demonstrate ability to use various evidence-based treatment approaches, including Motivational Interviewing and behavior change counseling.
  • Facilitate group sessions and complete biopsychosocial assessments.
  • Assist patients and families with navigation of local community resources and agencies.

Requirements

  • Bachelor's degree in Social Work from a recognized university.
  • Current registration with the Nova Scotia College of Social Workers.
  • 2 years' post BSW related clinical experience.
  • Previous experience working in community-based settings is preferred.
