We are seeking a skilled Social Worker to join our team at Fraser Health, providing comprehensive services and therapeutic clinical counseling to Indigenous clients and their families.
Key Responsibilities- Conduct comprehensive social work and psychosocial assessments of Indigenous clients and their families, using a variety of assessment tools and culturally appropriate indigenous practices.
- Provide therapeutic services, exploring the psychosocial response to identify problems, personal concerns, treatment objectives, and facilitating change in coping style, behavior, attitude, and feelings.
- Collaborate with the interdisciplinary team in client care planning, interviewing clients and families to assess and determine care plans and type of service required.
- Support a seamless approach to care and service delivery across the healthcare system, participating in the admission and discharge process of clients in acute care and community.
- Partner with community services, health, and social agencies to assess and manage individuals at risk, making referrals to appropriate resources when longer-term treatment or follow-up is required.
- Current full registration with the BC College of Social Workers.
- Two years' recent related experience in a complex healthcare environment, such as a community or home health setting, plus relevant related experience in an Indigenous environment/community.
- Valid BC Driver's License and access to a personal vehicle for business-related purposes.
- Comprehensive knowledge and understanding of counselling and clinical practice models as it relates to Indigenous individuals, families, groups, and communities.
- Demonstrated ability to conduct comprehensive social work and psychosocial assessments.
- Comprehensive counselling skills, including crisis counseling, brief therapeutic interventions, and loss and grief applicable to patients, families, and their support networks.
- Familiarity with traditional wellness practices, customs, values, and traditions of local First Nations.
