Specialty Pharmacy Clinical Care Coordinator

About the PositionWe are seeking a dedicated and ambitious Float Registered Practical Nurse to join our team. As a key member, you will be responsible for running and maintaining the infusion clinic, scheduling patient appointments, and delivering exceptional patient care. You will work closely with physicians, growing and maintaining relationships to ensure seamless patient experiences.
Your primary duties and responsibilities will include:
- Administering and training medications (IV, IM, SC) in patients' homes or specialty clinics settings
- Monitoring post-injection/infusion treatments and charting/reporting activities according to program protocols
- Providing patient education based on program protocols, either individually or in groups
- Ensuring regular, predictable availability to meet appointment scheduling needs, providing quick access to necessary therapies
- Coordinating daily with patients, case managers, physicians, scheduling, and other departments to meet established timelines for scheduling appointments, treatments, and communication of information
- Developing and sustaining relations with key stakeholders, clients, president, physicians, and other healthcare professionals
- Participating in telephone consultations and/or web-based meetings/training sessions with the president or reporting manager to share information and uphold professional practice standards
To excel in this role, you should possess:
- A valid provincial license from the College of Nurses as a registered practical nurse
- Healthcare provider CPR certificate
- Minimum 3 years' experience as a registered practical nurse (preferred)
- Experience administering injectable medications (IV, IM, SC)
- Background in critical care, emergency, acute care inpatient/outpatient, or homecare settings (considered assets)
- ACLS certificate required
- Proficiency in MS Excel, Outlook, and MS Word
- Excellent customer service skills in dealing with patients, physicians, and clients
- Strong written and verbal communication skills
- Able to work autonomously with little supervision as part of a larger remote team
- Able to travel to clinics and homecare appointments within a set geographical boundary
