About the Role

We are seeking a compassionate and dedicated Healthcare Professional to join our team as a Home Visit Specialist. In this role, you will be responsible for providing high-quality patient care in the comfort of their own homes.

Key Responsibilities

  • Visit patients in their homes for one-on-one visits to assess their physical, psychosocial, and environmental needs.
  • Develop and implement personalized care plans to meet the unique needs of each patient.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Administer medication as prescribed by the patient's physician and educate patients on proper home health care strategies.
  • Provide effective communication to patients/clients, their family members, and healthcare professionals.

Requirements

  • Graduate of an accredited Diploma, Associate, or Bachelor's School of Nursing, or vocational nursing.
  • Current license with College of Nurses Ontario and in Good Standing as an RN or RPN.
  • Professional Liability Insurance - Mandatory.
  • Proof of current CPR certification.
  • Compassionate, friendly demeanor and excellent communication skills.
