About the Role

We are seeking a skilled and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team as a Community Health Nurse. As a key member of our interdisciplinary care team, you will provide exceptional nursing services to patients in their homes, clinics, and other community settings.

Key Responsibilities
  • Develop and maintain effective care plans in collaboration with patients, families, and healthcare professionals.
  • Provide nursing services in a variety of settings, including in-home, clinic, and retirement home settings.
  • Communicate and liaise with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Perform other clinical duties as required.

Requirements
  • Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma.
  • Current College of Nurses of Ontario (CNO) registration in good standing.

  • Exceptional interpersonal skills.
  • A valid driver's license and vehicle.
  • Excellent English verbal and written communication skills.
  • CPR Certification.
  • Current immunization record including 2 step TB test.
  • To protect our patients, we require a current vulnerable sector check and a clear background check.
