About the Role:
This is an exciting opportunity to join ParaMed Home Health Care as a Registered Practical Nurse (RPN) in a community care setting. As a key member of our team, you will be responsible for providing high-quality nursing care to patients in their homes, working collaboratively with other healthcare professionals to ensure comprehensive care.
Key Responsibilities:
- Participate in scheduled visits to assess patient needs and provide care within your scope of practice.
- Develop and implement individualized care plans, working closely with patients and their families to ensure their needs are met.
- Provide education and support to patients and their caregivers on disease management, medication, and other health-related topics.
- Collaborate with other healthcare professionals, including RNs, to ensure seamless care coordination.
- Document patient interactions, assessments, and care plans in a timely and accurate manner.
Requirements:
- Registration with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse in good standing.
- Valid driver's license and access to a reliable vehicle.
- Experience in delivering community care is an asset.
- Reliability, flexibility, and a strong commitment to providing excellent patient care.
