Social Supports Coordinator

The Government of Prince Edward Island is seeking a Casual Social Supports Coordinator to join our team in delivering a range of specific supports and services to eligible persons and families. As a key member of our Social Development and Seniors team, you will provide direction and case management in delivering high-quality services.
Key Responsibilities:
• Collaborate with clients, their families, and support networks to develop and implement personalized case plans that maximize opportunities for full citizenship.
• Provide screening and initial application supports for clients requiring eligibility assessments.
• Conduct home and facility visits to promote a person-centered approach in supportive service provision.
• Contribute to a psychologically safe work environment and provide specialization in the assessment and delivery of services using a collaborative case management model.
• Facilitate the development of support plans for individuals and families requiring Social Assistance, Child Care Subsidy, or Seniors Independence.
• Implement person-centered collaborative case management by combining efficiency, commitment, care, and compassion.
• Determine eligibility in accordance with Program Act/Regulation, policies, and procedures.
• Provide leadership in the development and implementation of support plans to address complex support needs for individuals and families at risk.
• Analyze information to confirm accuracy and evaluate the need for follow-up in determining eligibility for service.
• Utilize the motivational interviewing approach to strengthen a person's motivation and commitment to change.
• Communicate effectively with clients, families, government agencies, non-government organizations, and service providers.
• Stay up-to-date on policies and procedures involved in providing high-quality services to clients in the areas of Social Assistance, Child Care Subsidy, and Seniors Independence Initiative.
• Apply a risk management approach to recognize potential problematic situations and take measures to avoid crisis situations.
Requirements:
• Strong understanding of social development principles and practices.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively with clients, families, and support networks.
• Strong case management and problem-solving skills.
• Ability to work in a fast-paced environment and prioritize tasks effectively.
• Strong knowledge of policies and procedures related to social assistance, child care subsidy, and seniors independence.
• Ability to maintain confidentiality and handle sensitive information.
