Community Mental Health Nurse Specialist

We are seeking a dedicated Community Mental Health Nurse Specialist to join our team at Fraser Health Authority. In this role, you will provide treatment and care management services to individuals with mental illness and concurrent disorders.
Job SummaryThis is an exciting opportunity to make a meaningful difference in the lives of our clients. As a Community Mental Health Nurse Specialist, you will work collaboratively with interprofessional teams to provide person-centered and recovery-oriented care. You will assess client needs, develop and implement comprehensive treatment plans, and collaborate with other healthcare professionals to ensure seamless care.
Responsibilities- Coordinate care for clients by collaborating with interprofessional care teams, performing intake functions, conducting mental health assessments, developing, implementing, maintaining, and adjusting comprehensive treatment plans based on client needs and best practices.
- Plan and provide direct nursing care to clients, including reviewing client/family history, conducting physical/psychosocial assessments, assessing vital signs, developing a care plan specific for the client, and collaborating with other members of the healthcare team to discuss and review the care plan.
- Establishes, maintains, and enhances therapeutic relationships based on respect with clients and their informal support networks by engaging stakeholders in treatment planning and evaluation, providing support and follow-up planning, promoting and supporting clients to manage self-care, encouraging informed decision-making, and empowering clients to improve their quality of life.
To be successful in this role, you will require:
- Graduation from an approved School of Nursing or Psychiatric Nursing.
- Two years recent related experience providing treatment services to individuals with mental health and/or concurrent disorders.
- Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Valid Class 5 BC Driver's License and access to personal vehicle with appropriate insurance coverage.
