Job Summary

We are seeking a Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team as a Community Nurse. As a Community Nurse, you will provide high-quality nursing care to patients in their homes, clinics, and communities. You will work collaboratively with our interdisciplinary care team to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities
  • Provide nursing care to patients in their homes, clinics, and communities
  • Develop and implement individualized care plans that meet the unique needs of each patient
  • Collaborate with our interdisciplinary care team to ensure seamless care coordination
  • Communicate effectively with patients, families, and healthcare providers to ensure optimal care outcomes
  • Participate in ongoing education and professional development to stay current with best practices and industry developments
Requirements
  • Current registration with the College of Nurses of Ontario (CNO)
  • Valid driver's license and vehicle
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and critical thinking skills
