About the Role

CarePartners is a leading provider of home health care services in Ontario, offering a range of nursing, personal support, and therapy services to patients of all ages.

We are seeking a skilled and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team in a community nursing role.

Key Responsibilities
  • Develop and maintain effective care plans for patients receiving community health services
  • Provide exceptional nursing services to patients in their homes, clinics, or other settings
  • Communicate and collaborate with patients, family members, and healthcare teams to ensure seamless care
  • Perform other clinical duties as required

Requirements
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • Strong interpersonal and communication skills
  • A valid driver's license and vehicle
  • CPR certification and current immunization record
