CarePartners is seeking a dedicated and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team as a Community Nurse. As a Community Nurse, you will have the opportunity to provide exceptional nursing services to patients in their homes, clinics, and other community settings.

Key Responsibilities
  • Develop and maintain effective care plans for patients receiving community health services
  • Provide nursing services to patients in various settings, including in-home, clinic, and retirement home settings
