Job Summary

We are seeking a dedicated Patient Care Coordinator to join our healthcare team at St. Boniface Hospital.

Salary Range:

$64,000 - $90,000 per year

Job Description

The Patient Care Coordinator will provide professional nursing services for patients with complex and unpredictable health needs. This includes independent nursing services, administration of prescribed medical treatments, and coordination of patient care and discharge.

Responsibilities

  • Recent acute Nursing experience is required.
  • Graduate of an approved Registered Nursing education program is required.
  • Current CRNM registration is required.
  • Active participation in professional associations e.g. Association of Regulated Nurses of Manitoba (ARNM) is preferred.

Qualifications and Skills

  • Scope of practice as documented in the Regulated Health Professions Act (RHPA).
