Healthcare Leader for Long-Term Care

We are seeking a dedicated and compassionate Nurse Practitioner to join our Steeves and Rozema Group team. As a healthcare leader in long-term care, you will be responsible for providing high-quality patient care, collaborating with healthcare teams, and promoting a positive work environment.
About the RoleThis is a full-time position that involves working Monday to Friday. We offer a competitive salary of $90,000 - $110,000 per year, depending on experience, as well as a comprehensive benefits package including dental care, disability insurance, extended health care, flexible schedule, life insurance, on-site parking, and vision care.
Job DescriptionThe successful candidate will possess current registration and good standing with the College of Nurses of Ontario, as well as Nurse Practitioner Certification from a recognized program. Experience in a community and/or primary care setting is an asset, and knowledge of RNAO Best Practice guidelines and MOH Regulations and Act is essential.
The ideal candidate will have demonstrated competency in advanced assessments, diagnoses, and prescribing pharmaceuticals and non-pharmacological interventions within the legislated scope of practice. They will also possess excellent interpersonal, communication, organization, and team skills, as well as the ability to work independently and collaboratively as part of a healthcare team.
In this role, you will perform assessments, diagnose, treat, and provide health education/counselling to residents. You will also contribute to chart documentation, participate in chart reviews, and case conferences, and facilitate continuity of care by working collaboratively with professional healthcare teams.
Key Responsibilities Include:- Performing assessments, diagnosis, treatment, health education/counselling, and other clinical activities according to CNO standards of practice for NPs
- Contributing to chart documentation, participating in chart reviews, and case conferences
- Facilitating continuity of care by working collaboratively with professional healthcare teams
- Providing primary or shared medical care, consultation, and/or follow-up to residents and their families
- Current registration and good standing with the College of Nurses of Ontario
- Nurse Practitioner Certification from a recognized program
- Experience in a community and/or primary care setting (asset)
