About the Role- Provide clinical follow-up and care to patients in accordance with their primary care plan and personal goals of care.
- Develop and provide formal education to patients, families, and the general public on health-related topics.
- Work collaboratively with the interprofessional team to provide comprehensive care and services to individuals, groups, and communities.
Northern Health Authority covers an area of nearly 600,000 square kilometers and offers health services in over two dozen communities and 55 First Nation's communities. We deliver hospital and community-based health care for a population of 300,000.
