Job Summary

We are seeking a motivated Licensed Practical Nurse with a dedication for delivering compassionate patient care to join our team in Primary Care. The successful candidate will function as a member of the interprofessional team and apply best practice to provide competent, safe, and ethical care for people and their families in settings such as physician practices, homes, schools, and communities.

Key Responsibilities

Provide clinical follow up according to the person's primary Care Plan and the person's personal goals of care to inform plans of care developed collaboratively with the interprofessional team.
Support people and their families to achieve and maintain optimal health.
Perform assessments, plans, and implements and provides personal and nursing care to people.
Administer oral, subcutaneous and intramuscular medications in accordance with pharmaceutical and organizational policies and procedures.

Qualifications

Graduation from a recognized program for Practical Nurses with one (1) year of recent, related experience; or an equivalent combination of education, training and experience.
Registration with BC College of Nurses and Midwives as a practicing LPN registrant (BCCNM).
Current, valid BC Driver's License.

Skills and Abilities

Demonstrated knowledge and skills within the LPN defined scope of practice or patient population dependent on the nature of the caseload (acute care, gerontology, palliative, etc.).
Ability to communicate effectively, both verbally and in writing.
Ability to establish and maintain rapport with people and their families.
Ability to work independently and collaboratively with others within the context of an interprofessional team.
Ability to observe and recognize changes in a person's condition or health status.
Ability to plan, organize and prioritize; and ability to implement a plan of care.
Ability to operate related equipment including proficiency in relevant computer applications.
Physical ability to perform the duties of the position.
