Job Summary

At Northern Health Authority, we are seeking a skilled Community Health Worker II to join our team. As a key member of our interprofessional healthcare team, you will provide home support services to clients in accordance with a pre-established care plan. Your responsibilities will include assisting with activities of daily living, promoting maximum independence, and ensuring the well-being of individuals and families in the community.

Key Responsibilities

• Provide home support services to clients in accordance with a pre-established care plan
• Assist with activities of daily living, such as bathing, grooming, and oral hygiene
• Promote maximum independence and well-being of individuals and families in the community
• Work collaboratively with the interprofessional healthcare team to ensure high-quality patient care

Requirements

• Graduation from a recognized Home Support Attendant/Resident Care Aid Program
• Registration with the BC Care Aid & Community Health Worker Registry
• Valid Class 5 BC Drivers' License
• Certificates in CPR, First Aid, and Food Safe
• Use of personal vehicle
• Or equivalent combination of current education, training, and experience
