Social Worker II Position

We are seeking a highly skilled and compassionate Social Worker II to join our team at Northern Health Authority. As a key member of our healthcare team, you will provide comprehensive assessment, care planning, and treatment coordination for designated clients and their families throughout the continuum of care.
Key Responsibilities
• Provide direct psychosocial care, education, navigation, and advocacy to maintain or improve the health and social well-being and functioning of patients/clients/residents and their families in the Quesnel Health Services Delivery Area.
• Collaborate with the interdisciplinary team to provide comprehensive assessment, care planning, and treatment coordination for designated clients and their families.
• Develop and implement individualized care plans that address the unique needs and goals of each client and their family.
• Provide education and support to clients and their families on health and social issues, and connect them with community resources and services as needed.
• Work collaboratively with other healthcare providers and the multidisciplinary team to ensure seamless care coordination and communication.
Requirements
• Bachelor's Degree in Social Work from an accredited university.
• Two (2) years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training, and experience.
• Current registration with the British Columbia College of Social Workers.
• Current valid BC driver's license.
