Job Summary:
We are seeking a highly skilled and compassionate Social Worker, BSW to join our team at Fraser Health. As a Social Worker, you will play a vital role in providing psychosocial/emotional assessment and therapeutic clinical counselling services to clients in our assigned clinical areas.
Key Responsibilities:
- Conduct psychosocial/emotional assessments of clients using various methods, including focused interviewing and data collection.
- Provide basic therapeutic clinical counselling services to clients based on a psychosocial assessment.
- Develop and implement care plans that address the psychosocial/emotional components of illness, injury, and disease.
- Collaborate with clients, healthcare professionals, and community agencies to provide education and consultation on social work practice and area of clinical specialty.
- Maintain client records and provide input to the interdisciplinary team in client care planning and discharge planning.
Requirements:
- Bachelor's Degree in Social Work from an approved school of Social Work.
- Current full registration with the BC College of Social Workers (BCCSW).
- Knowledge and understanding of psychosocial/emotional issues related to illness/injury/disease, and current social work treatment modalities.
- Ability to communicate effectively verbally and in writing with clients, healthcare professionals, and community agencies.
