Job Overview

We are seeking a Certified Healthcare Professional - Patient Care Specialist to join our team. In this role, you will have the opportunity to work in a fast-paced environment and make a meaningful impact on the lives of our patients.

About the Role

This position requires a Registered Practical Nurse (RPN) with a strong patient-focused approach. You will provide culturally competent care to patients with complex needs, assess their conditions, and report any changes to the appropriate team members.

You will also administer medications according to policies and procedures, document therapeutic interventions, and act as a patient advocate. Additionally, you will participate in continuous quality improvement activities and contribute to creating a world-class health experience for patients and their families.

Requirements
  • Current Certificate of Registration in the General Class with the College of Nurses of Ontario
  • Current Basic Cardiac Life Support (BCLS) certification
  • Coronary Care Course 1, or commitment to completing within 1 year of date of hire
  • Proven clinical experience with patient-focused care, including strong patient assessment, care planning, patient education, and documentation skills
Preferred Qualifications
  • Practical Nurse Diploma from an approved nursing program
  • Membership in the Registered Practical Nurses' Association of Ontario (RPNAO)
  • At least 1 year of recent nursing experience working as a Registered Practical Nurse in an acute care setting within the past 3 years
  • Experience working with electronic medical records
Compensation

The estimated salary for this position is $65,000-$80,000 per year, depending on experience.
