Job Summary

We are seeking a skilled Mental Health Therapist to join our team. As a Mental Health Therapist, you will work closely with patients to provide individualized treatment plans, education, and ongoing assessment of progress. You will also facilitate group-based education and process-based content, and collaborate with other stakeholders to develop and implement aftercare plans.

Key Responsibilities
  • Provide individualized treatment plans, education, and ongoing assessment of progress to patients using evidence-based modalities.
  • Facilitate group-based education and process-based content, including abstinence-based relapse prevention.
  • Collaborate with other stakeholders to develop and implement aftercare plans, including referrals to abstinence-based recovery programs.
  • Prepare comprehensive discharge summaries and referent reports in a timely manner.
  • Complete detailed documentation and charting in accordance with relevant College standards.
  • Liaise with patients, family members, referral sources, community providers, and other members of the circle of care.
  • Engage in personal and professional development, maintain professional associations, and provide current documentation to the Human Resources Department.
  • Undertake Quality Assurance (QA) activities as assigned, including completion of outcome measures.
Requirements
  • Completion of a Master's degree program in Counselling or Social Work (preferred).
