Job Summary

We are seeking a skilled Mental Health Therapist to join our team. As a Mental Health Therapist, you will work closely with patients to provide individualized treatment plans, education, and ongoing assessment of progress. You will also facilitate group-based education and process-based content, identify treatment goals, and coordinate with broader clinical teams.

Key Responsibilities
  • Provide individualized treatment plans, education, and ongoing assessment of progress to patients using evidence-based modalities.
  • Facilitate group-based education and process-based content.
  • Identify treatment goals and coordinate with broader clinical teams.
  • Deliver high-quality psychoeducational and process groups using evidence-based modalities.
  • Coordinate case management and communication across inter-professional teams.
  • Collaborate with stakeholders to develop, implement, and follow-up on patient aftercare plans.
  • Prepare comprehensive discharge summaries and referent reports.
  • Complete detailed documentation and charting in accordance with relevant College standards.
  • Liaise with patient's family, referral sources, community providers, and other members of the circle of care.
Requirements
  • Master's degree in Counselling or Social Work (preferred).
