The Licensed Practical Nurse (LPN) is a key member of our care team, responsible for delivering high-quality, resident-centered care in our long-term care facility. As a LPN, you will work closely with our interdisciplinary team to assess resident needs, develop and implement individualized care plans, and provide direct care to our residents.
Responsibilities- Assess resident needs and develop individualized care plans in collaboration with the interdisciplinary team.
- Provide direct care to residents, including administering medications, performing wound care, and providing emotional support.
- Liaise with physicians, community agencies, and families to ensure seamless care coordination.
- Participate in interdisciplinary care conferences to review resident progress and adjust care plans as needed.
- Document resident care and progress in a timely and accurate manner.
- Support the administrative requirements of the community, including admissions and discharge processes.
- Actively participate in orientation of new staff, community programs, and educational in-services.
- Graduate of an approved program for Practical Nursing and current licensure with the British Columbia College of Nurses and Midwives (BCCNM) as an LPN.
- Valid Occupational First Aid Level 1 certification.
- Successful annual Resident Assessment Instrument (RAI) competency testing.
- Previous experience in a long-term care facility an asset.
- Ability to read, write, and speak English proficiently and understand verbal and written instructions.
- Computer skills, including the ability to navigate basic computer programs such as Word, Excel, and Outlook.
- Demonstrates collaborative and team working skills.
