Salary: $32 per hour (commensurate with experience)
Job Description:
We are seeking an experienced Community Care Navigators Lead to join our team. As a key role in the Caregiver Rx Social Prescribing Project, you will be responsible for providing individualized support to family caregivers, helping them manage their practical concerns and ongoing needs that contribute to their physical, social, and emotional wellbeing. This includes providing emotional support, assistance with navigating health and social services, and connecting caregivers with community resources tailored to their needs. You will build meaningful ongoing relationships with caregivers, co-create wellness plans, and use motivational interviewing and action planning to empower caregivers in reaching their wellness goals. You will also play an integral role in developing and maintaining referral pathways into our program within the wider health and social care community.
Key Responsibilities:
• Provide empathetic 1-1 emotional support to caregivers in response to incoming referrals.
• Assist with problem solving and creating action plans where appropriate, using a Brief Action Planning approach.
• Assist with accessing services/resources where appropriate.
• Liaise with community programs and maintain outgoing referral and feedback pathways.
• In collaboration with team members, regularly update community resources.
• Maintain client record data in a timely and accurate manner via the FCBC database (Salesforce-NonProfit based).
Requirements:
• Degree in healthcare field, social work, social services, geriatric care, mental health or other related discipline, or a combination of education and experience
• ~2 years case management experience (or similar) supporting individuals and/or family caregivers.
• Excellent interpersonal and communication skills (verbal and written), with a demonstrated ability to provide empathetic, warm, and professional support that is accepting and inclusive of all clients.
• Experience applying motivational interviewing techniques to support others is an asset.
• Experience working with groups an asset, with facilitation skills, preferably in a workshop environment. Presentation skills an asset.
• Proficient in Microsoft Office: Word, Excel, Outlook and must have a high level of comfort using online web platforms and digital tools (and the ability to simplify processes for technologically unskilled users).
• Familiarity with databases such as Salesforce (or similar CRM systems) an asset.
