Community Health Nurse

CarePartners is seeking a skilled and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team as a Community Health Nurse. As a Community Health Nurse, you will have the opportunity to work in a variety of settings, including in-home, clinics, retirement homes, and schools, providing exceptional nursing services to patients of all ages.
Key Responsibilities- Develop and maintain effective care plans for patients receiving community health services, working independently or in collaboration with Home and Community Care Support Services (HCCSS).
- Provide nursing services to patients in-person and virtually, ensuring exceptional patient care and outcomes.
- Communicate and liaise with patients, family members, and other members of the interdisciplinary care team to ensure seamless care coordination.
- Perform other clinical duties as required, including developing and implementing care plans, conducting assessments, and providing education to patients and families.
- Current registration with the College of Nurses of Ontario (CNO) in good standing.
- Strong desire and commitment to making a difference in the lives of patients.
- Exceptional interpersonal skills, with the ability to communicate effectively with patients, families, and healthcare teams.
- A valid driver's license and vehicle, as regular travel is required.
- Excellent English verbal and written communication skills.
- CPR certification and current immunization record, including a 2-step TB test.
- A current vulnerable sector check and clear background check, as required by CarePartners.
