Nurse Practitioner

We are seeking a highly skilled and dedicated Registered Nurse to join our team as a Primary Care Nurse. As a key member of our interprofessional team, you will provide high-quality, patient-centered care to individuals, families, and communities in a variety of settings.
Key Responsibilities- Provide clinical follow-up and care to patients in accordance with their primary care plan and personal goals of care.
- Develop and implement individualized care plans in collaboration with the interprofessional team.
- Provide education and support to patients, families, and communities on health-related topics and promote healthy behaviors.
- Collaborate with other healthcare professionals to ensure seamless care transitions and coordination.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Current registration with the BC College of Nurses and Midwives as a practicing RN or RPN registrant.
- Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings.
- Current valid driver's license.
- Experience in developing and providing formal education to patients, families, and the general public.
- Knowledge of adult learning principles and facilitation skills to effectively educate patients and others about health-related topics.
