Nurse Practitioner

Fraser Health is seeking a skilled Licensed Practical Nurse to join our team at WILLIAM RUDD HOUSE in New Westminster, BC. As a key member of our multidisciplinary team, you will provide high-quality nursing care to patients and residents, working collaboratively with other healthcare professionals to achieve optimal outcomes.
Responsibilities
- Conduct comprehensive assessments of patients' and residents' physical, emotional, and social needs, developing and implementing individualized care plans to address these needs.
- Provide direct patient care, including administering medications, performing procedures, and providing emotional support and education to patients and their families.
- Collaborate with other healthcare professionals to coordinate care and ensure seamless transitions between departments and levels of care.
- Participate in quality improvement initiatives and education sessions to enhance skills and knowledge, contributing to a culture of continuous learning and improvement.
- Perform other related duties as assigned, working collaboratively with the healthcare team to achieve shared goals and objectives.
Education and Experience
Graduate of a recognized practical nursing program or equivalent combination of education, training, and experience. Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
Skills and Abilities
- Ability to communicate effectively, both verbally and in writing.
- Ability to deal with others effectively.
- Ability to organize work and implement a plan of care.
- Knowledge and competencies to work at full scope of practice as currently required by the BCCNM, including administering medications.
- Ability to operate related equipment.
- Physical ability to carry out the duties of the position.
