We are seeking a skilled Advanced Clinical Care Specialist to join our team of over 35 clinicians. This part-time permanent position offers the opportunity to work on a variety of medical cases and collaborate with interdisciplinary teams.

About the Role
  • Deliver exceptional patient care as a key member of our clinical team
  • Collaborate with Medical Directors to manage complex chronic conditions in LTC residents
  • Provide high-quality customer service and support to clients and staff
  • Pursue ongoing education and professional development opportunities

This is an excellent opportunity for experienced Nurse Practitioners to take their skills to the next level and advance their careers. We offer a competitive salary range of $75-$80 per hour and flexible scheduling options.

Requirements
  • Current extended class certification with College of Nurses of Ontario
  • Experience with chronic disease management and geriatric care
