Job Summary
The Fraser Health Authority is seeking a Community Health Nurse to join our team. As a Community Health Nurse, you will work with clients in their homes and communities to provide nursing care and support. This is a casual opportunity at Queen's Park Care Centre in New Westminster, BC.
About the Role
As a Community Health Nurse, you will work independently in the community setting, assessing and treating clients with complex health conditions. You will work collaboratively with an interprofessional team to provide client-centered care, promoting health, dignity, and independence. You will be responsible for assessing clients' needs, developing care plans, and providing direct client care, including teaching and counseling. You will also facilitate care conferencing to review client care plans and ensure seamless transitions across the care continuum.
Responsibilities
- Conduct initial and ongoing client assessments to identify health needs and develop care plans.
- Provide direct client care, including teaching, counseling, and support to promote health and independence.
- Collaborate with the interprofessional team to develop and implement care plans.
- Facilitate care conferencing to review client care plans and ensure seamless transitions across the care continuum.
- Develop and maintain accurate and comprehensive clinical records.
- Maintain professional practice growth, knowledge, and expertise to reflect current standards of practice.
Qualifications
Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). One year recent related clinical experience assessing and treating complex geriatric and/or adult patients with chronic health conditions in an acute or community/outpatient care setting. Valid BC Driver's license and access to a personal vehicle for business-related purposes.
