Job Title: Mental Health Support Worker

We are seeking a highly motivated and compassionate Mental Health Support Worker to join our team at Heritage Nanny Community Living and Support Inc. As a key member of our team, you will play a vital role in providing support and services to individuals with mental health needs.

Job Summary:

The Mental Health Support Worker will work closely with clients, families, and community agencies to provide a range of services, including case management, counseling, and support. This is a dynamic and rewarding role that requires a strong commitment to providing high-quality services and supporting individuals in achieving their goals.

Key Responsibilities:
  • Provide individual and group counseling sessions to clients
  • Develop and implement rehabilitation programs to support clients in achieving their goals
  • Conduct assessments and evaluations to identify client needs and develop support plans
  • Work with clients to develop life skills and provide support in daily living activities
  • Collaborate with community agencies and healthcare providers to ensure comprehensive support for clients
  • Provide crisis intervention and support to clients in times of need
  • Develop and maintain relationships with clients, families, and community agencies
  • Participate in team meetings and contribute to the development of service plans
  • Maintain accurate records and reports of client interactions and progress
Requirements:
  • College/CEGEP diploma in a related field (e.g. social work, psychology, counseling)
  • 2 years of experience working in a mental health or social services setting
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and crisis management skills
  • Ability to maintain confidentiality and handle sensitive information
