Job Title: Mental Health Support Worker

We are seeking a skilled Mental Health Support Worker to join our team at Heritage Nanny Community Living and Support Inc. The successful candidate will provide support and services to individuals with mental health needs, promoting their well-being and independence.

Key Responsibilities:
  • Prepare reports and case histories to document client progress and needs
  • Administer and coordinate the delivery of specific services within the community
  • Assess client needs and eligibility for services, developing rehabilitation programs to meet their requirements
  • Provide suicide and crisis intervention, resolving conflict situations and supervising activities of clients in group homes and half-way houses
  • Liaise with other social services agencies and healthcare providers to ensure comprehensive support for clients
  • Develop and maintain program statistics for evaluation and research purposes
Requirements:
  • 2 years of experience in a related field, preferably in mental health support or rehabilitation
  • Excellent interpersonal and communication skills, with the ability to work effectively with clients, staff, and community partners
  • Flexibility and adaptability in a fast-paced environment
  • Ability to maintain confidentiality and handle sensitive information
