Primary Health Care Nurse Practitioner

We are seeking a highly skilled and dedicated Nurse Practitioner to join our team at the Lloydminster Primary Care Network.
About the Role:The successful candidate will work collaboratively with a multi-disciplinary team to provide comprehensive health services to individuals across their lifespan. As a Nurse Practitioner, you will be responsible for assessing, diagnosing, ordering and interpreting diagnostic tests, prescribing treatment, and performing procedures to manage and meet the health needs of patients.
Key Responsibilities:- Provide primary health care services to individuals and families, including health promotion, disease prevention, and management of acute and chronic conditions.
- Collaborate with other healthcare professionals to ensure comprehensive and coordinated care.
- Develop and implement individualized care plans, taking into account the unique needs and circumstances of each patient.
- Conduct thorough assessments, including physical examinations, medical histories, and laboratory tests, to diagnose and manage health conditions.
- Prescribe medications and treatments, as well as perform procedures, to manage and treat health conditions.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care and coordination.
- Master's degree in Nursing or related field.
- Current registration with the College of Registered Nurses of Alberta (CRNA) and the College of Registered Nurses of Saskatchewan (CRNS).
- Provisional Graduate Nurse Practitioner licensure.
- Current BLS and CPR certification.
- Working knowledge of healthcare IT systems, including Alberta Netcare and Clinic EMRs.
- Desktop IT skills with Microsoft Office (Outlook, Word, Excel).
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and healthcare teams.
- Competitive salary range: $120,000.00-$140,000.00 per year.
