Senior Occupational Therapist

Role Summary
We are seeking an experienced Occupational Therapist to join our team at Fraser Health. The successful candidate will work in a community or home health setting, providing occupational therapy services to patients and collaborating with healthcare professionals to facilitate timely discharge planning.
Key Responsibilities
- Assess patient needs and develop individualized care plans to promote functional independence and occupational performance
- Collaborate with community service providers and healthcare teams to identify and address patient care needs
- Develop and implement strategies to facilitate patient discharge and transition to community care
- Prescribe and recommend adaptive equipment and provide education to patients and caregivers on independent living skills and occupational performance
- Participate in quality improvement activities and maintain current knowledge in evidence-based occupational therapy
Requirements
- Current full registration with the College of Occupational Therapists of British Columbia (COTBC)
- Two years' recent experience in a community or home health setting, or an equivalent combination of education and experience
- Valid BC Driver's License and access to a personal vehicle for business-related purposes
- Excellent communication and leadership skills, with the ability to work effectively in a team-based environment
- Ability to prioritize workload and problem-solve in a fast-paced environment
