Job Summary

We are seeking a highly motivated and compassionate Peer Support Worker to join our team at Fraser Health Authority. As a Peer Support Worker, you will play a vital role in supporting individuals with mental health conditions and/or substance use experiences to live successfully in the community.

Key Responsibilities
  1. Establish and maintain positive relationships with clients, providing emotional and social support to help them achieve their goals.
  2. Collaborate with clients and the CRESST team to develop and implement personalized care plans that address their unique needs and circumstances.
  3. Provide information and resources to clients, empowering them to make informed decisions about their health and well-being.
  4. Assist clients with community integration activities, such as accessing healthcare services, social services, and recreational activities.
  5. Work with clients to develop life skills, such as personal hygiene, meal planning, and financial management.
  6. Participate in program planning, development, and evaluation activities to ensure the delivery of high-quality services.
  7. Maintain accurate and up-to-date records of client interactions and progress.
Requirements
  • Lived experience of mental illness or substance use.
  • Grade 12 completion and a recognized peer support worker training program.
  • One year of recent related work experience in mental health, addictions, homelessness, and/or social services.
  • Food Safe Certificate and a valid Class Five BC Driver's License.
