Continuing Care Coordinator

The Care Coordinator is responsible for assessing, planning, and authorizing services for clients referred to Continuing Care. They help provide an array of services to assist individuals and families cope with complicated health situations in the community.
Care Coordinators assist clients to identify their goals, unmet needs, and resource requirements. Based on the assessment, they assist the client and their care partners to formulate a plan to meet identified goals.
They work with interprofessional teams to facilitate hospital discharge to return to the community. Care Coordinators are fiscally responsible for the care, services, and equipment they authorize within available Department of Seniors and Long Term Care policies and programs.
About YouWe are seeking a skilled professional with a Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies. The ideal candidate will have current and ongoing registration with the applicable Nova Scotia Professional Regulatory body and a minimum of three years' experience.
They will have demonstrated knowledge and/or experience in a community or long-term care setting, as well as knowledge and/or experience in dedicated case management or discharge planning. The candidate will be competent with computer use, keyboarding, and able to conduct assessments using a laptop.
They will also have excellent assessment, interviewing, and negotiation skills, as well as excellent verbal and written communication skills. The ability to work independently and co-operatively in a busy multidisciplinary situation is essential.
HoursFull-time, long-assignment; 70 hours bi-weekly. Shifts may include days, evenings, weekends, and holidays.
Compensation and Benefits$41.06 - $46.75 Hourly. Successful candidates may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
