Physical Therapist

Vancouver Coastal Health is seeking a skilled Physical Therapist to join our team at the Qathet General Hospital in Powell River, BC. As a Physical Therapist with our organization, you will have the opportunity to work in partnership with clients and families to establish goals that are safe, realistic, and reasonable.
Key Responsibilities- Assess service eligibility and facilitate the development, implementation, and adaptation of a care plan.
- Deliver direct care and provide care management to assist clients to manage their own care and navigate through the various services available within the community setting.
- Work in consultation with other system partners and incorporates current evidence into practice.
Home Health programs provide a range of health care and support services to people living in the community who have acute, chronic, palliative or rehabilitative health care needs.

Qualifications- Current full registration with the College of Physical Therapists of BC (CPTBC).
- Recent, related experience in Home Health clinical practice in Community.
- Valid BC Driver's License required as local area travel may require the use of a personal vehicle.
