About the Role

VHA Home HealthCare is a leading provider of home care services in Ontario, with a team of over 3,000 dedicated professionals. As a Home Health Care Specialist, you will play a vital role in delivering high-quality, client-centered care to individuals in their homes, schools, retirement homes, and community settings.

Key Responsibilities
  • Assess and monitor clients' needs, developing and implementing personalized care plans to ensure optimal outcomes.
  • Deliver a range of clinical procedures, including pneumostat care, atrium dry seal care, T-tube care, IV Lasix administration, and other specialized medical interventions.
  • Evaluate care plan interventions through ongoing reassessment and evaluation results, ensuring the quality of care and identifying areas for improvement.
  • Support client safety and well-being through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
  • Collaborate with nursing supervisors and case coordinators to ensure seamless care coordination and communication.
Requirements
  • Valid driver's license and reliable vehicle for traveling.
  • Acute Care and Medical-Surgical nursing experience is an asset.
  • Knowledge of community nursing is an asset.
  • Commitment to lifelong learning and professional growth, with the ability to apply critical thinking skills to provide client care.
