Job Summary:

The successful candidate will provide advanced nursing care to patients in a primary care setting, working autonomously and as part of a multidisciplinary team. The ideal candidate will have a strong background in clinical nursing, excellent communication skills, and the ability to work effectively in a fast-paced environment.

Responsibilities:
  • Provide advanced nursing care to patients, including health assessments, diagnostics, and pharmacotherapeutics.
  • Work autonomously and as part of a multidisciplinary team to provide comprehensive care to patients.
  • Develop and implement care plans, and evaluate patient outcomes.
  • Collaborate with other healthcare professionals to ensure seamless care delivery.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements:
  • Master's degree in Nursing, Nurse Practitioner Stream or equivalent.
  • Minimum of five years recent clinical nursing experience, preferably in primary care.
  • Experience working with clients with mental health and/or addictions an asset.
  • Active registration on the RN Extended Practice Register of the College of Registered Nurses of Manitoba.
Preferred Qualifications:
  • Experience working across sectors and linking between programs/organizations.
  • Two years of practice in the extended practice role in community health and primary care.
  • Competency in managing technology in healthcare systems.
