Clinical Pharmacy Specialist in Ambulatory Care

At PeaceHealth, we are seeking a skilled Clinical Pharmacy Specialist to join our team in an ambulatory care setting.
Job SummaryThis role involves collaborating with medical group clinicians and staff to deliver coordinated, high-quality patient care, focusing on medication management.
- Serving as a key member of various committees within the medical group, providing consultation and expertise as requested.
- Designing and delivering education and training programs for team members and patients on drug therapy.
- Oversighting the orientation and training of pharmacy students, residents, and technicians.
This is a full-time opportunity that requires:
- A Bachelor of Science degree in Pharmacy from an ACPE-accredited institution.
- Possession of a Doctor of Pharmacy degree or equivalent qualifications.
- Comprehensive knowledge of evidence-based clinical pharmacy practice in ambulatory care settings.
- Minimum 3 years of experience in advanced pharmacotherapy specialist practice in an ambulatory clinical pharmacy setting, with a focus on direct patient care, medication management, consulting, and teaching.
- Certification in Board of Pharmacy Specialties related to ambulatory care (BCACP, BCGP, etc.), Anticoagulation Certification, ASHP Anticoagulation Certificate, or University of South Indiana Anticoagulation Therapy Management Program Certificate.
- Basic Life Support certification within 90 days of hire.
