Job Summary

We are seeking a highly skilled and experienced Clinical Coordinator to join our Cardiac Care Program at Southlake Regional Health Centre. As a key member of our interprofessional team, you will be responsible for coordinating efficient patient flow activities across the program, ensuring quality of patient care delivery, and implementing evidence-based practices that are sensitive to limited resources within a patient-centred care philosophy.

Responsibilities
  • Collaborate with the Triage coordinators, and PFN/Charges of the cardiac program to ensure efficient patient flow activities
  • Oversee daily staffing, ensure quality of patient care delivery, and patient flow related activities within the program
  • Lead the team in implementing evidence-based practices that are sensitive to limited resources within a patient-centred care philosophy
  • Collaborate with the manager regarding human resource responsibilities, including recruitment, just in time teaching, and ongoing performance evaluation
  • Liaise with members of the inter-professional team to ensure excellence in patient care through monitoring of standards of care and patient outcomes
  • Participate in quality assurance opportunities and support accountability for providing high-quality care
  • Collaborate with the interprofessional teams and PFAs to create the ultimate patient experience
Qualifications
  • RN Certificate of registration from the College of Nurses of Ontario
  • Baccalaureate Degree in Nursing Science (preferred)
  • Minimum of 2-5 years recent and relevant cardiac care experience
  • CNO, RNAO, other professional affiliations as appropriate
  • Demonstrates knowledge in clinical practice in the Cardiac Program
  • Demonstrates the ability to communicate and work with all members of the interprofessional team
  • Demonstrates the ability to problem solve clinical issues with the support of the manager
  • Demonstrates a strong patient and customer-focused philosophy in all interactions
  • Possess the ability to implement strategies that effectively facilitate change
  • Demonstrates clinical decision-making and organizational skills
  • Demonstrated excellence in communication, both written and verbal
  • Demonstrated problem assessment skills
  • Demonstrated commitment to customer service and teamwork
  • Demonstrated individual leadership skills
