Job Opportunity:

We are seeking a highly motivated and energetic Health Care Aide to join our Lethbridge Home Assist Peritoneal Dialysis Program.

The ideal candidate will have the ability to work collaboratively as part of a multi-disciplinary team, providing personal assistance to patients who require short-term assistance or ongoing support.

About the Role:

As a Patient Care Coordinator, you will work closely with patients, families, and caregivers to provide support services, including assisting with activities of daily living, hygiene, nutrition, mobility, recreation, and safety needs.

You will also be responsible for observing and reporting on patients' moods, health, and wellbeing, while maintaining effective communication with healthcare professionals.

Responsibilities:
  1. Provide personal assistance to patients, including supporting activities of daily living
  2. Assist with organized activities or programs
  3. Observe and report on patients' moods, health, and wellbeing
  4. Maintain effective communication with healthcare professionals
Requirements:

To be successful in this role, you must have:

  • Completion of a Health Care Aide Certificate within 18 months of hire
  • Active directory status on the Alberta Health Care Aide Directory
  • A confirmation letter as proof of enrolment from the Directory

Prior experience working in peritoneal dialysis or hemodialysis is an asset.

Salary and Benefits:

This is a full-time opportunity with a competitive salary range of $25.00 - $30.00 per hour, depending on experience.
