Job Summary

AVI Health and Community Services is seeking a skilled and compassionate Safer LPN/RN/RPN to join our team in promoting the health, dignity, and well-being of individuals affected by HIV, HCV, and substance use. As a key member of our interdisciplinary team, you will provide participant-centered healthcare, including assessment, screening, and education, to support individuals accessing our Safer program.

Key Responsibilities
  • Provide care and support for individuals with high levels of poly-substance use and complex health needs, using a harm reduction and trauma-informed approach.
  • Work collaboratively with the Safer team to develop and maintain individualized care plans, providing ongoing monitoring and support to participants.
  • Provide education, information, and referrals to participants, focusing on their self-identified needs and health status.
  • Collaborate with community partners to ensure seamless support for mutual participants.
  • Administer medications as ordered by program physicians and according to clinical protocols.
Requirements
  • Registered Nurse, Registered Psychiatric Nurse, or Licensed Practical Nurse with a minimum of 1-5 years of full scope, outreach nursing experience working with individuals with high levels of poly-substance use and mental health challenges.
  • Previous experience working in supervised consumption/overdose prevention services, safer supply, or other harm reduction programs.
  • Demonstrated commitment to social justice principles and practices, including harm reduction, anti-racism, decolonization, and combating stigma related to drug use, HIV, HCV, and gendered oppression.
  • Strong analysis of colonialism and intergenerational impacts of historical and ongoing colonization.
  • Excellent communication, conflict resolution, and crisis management skills.
