Mental Health and Addictions Outreach Nurse

Vancouver Coastal Health is seeking a skilled Registered Nurse or Registered Psychiatric Nurse to join our Intensive Care Management team as a Mental Health and Addictions Outreach Nurse in Sechelt, BC.
Key Responsibilities- Conduct comprehensive assessments of clients' mental and physical health needs, identifying priority care requirements and developing effective care plans.
- Provide counseling and support to clients, helping them to reduce their reliance on acute and community healthcare services.
- Assess high-risk situations and provide crisis intervention, ensuring the safety and well-being of clients.
- Collaborate with interdisciplinary teams to develop and implement client care plans, ensuring seamless continuity of care across the healthcare continuum.
- Facilitate the movement of clients between acute care, community agency support sites, and designated health centers.
- Participate in case conferences and meetings with multidisciplinary teams, ensuring effective communication and coordination of care.
- Assist clients with accessing necessary services, such as medical appointments.

Requirements- Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Diploma in Nursing or Psychiatric Nursing.
- Two (2) years' recent related experience working with individuals with mental health and addictions issues, or an equivalent combination of education, training, and experience.
- Valid BC Driver's License, with local area travel requiring the use of a personal vehicle.
